Report Service Hours
Any volunteer work for Children's Center for the Visually Impaired (CCVI) or at Alphapointe should be entered here. Please enter hours completed from May 16, 2009 to May 15, 2010. Hours should be rounded to the nearest hour and travel time does not count.
All fields are required. Please provide a valid email address so that we can contact you if we have any questions.
Examples of service: Working with visually impaired individuals at CCVI or Alphapointe, serving in a leadership position at CCVI or Alphapointe, or participating in a CCVI or Alphapointe fundraiser such as the CCVI Trolley Run or the CCVI Cookbook.
These volunteer hours do not count as Service for Sight hours: Participating in Tables That Bloom or the Poinsettia Sale.
